Applications for Flourish @LWJ are welcomed from schools who have carefully considered the nature of the provision on offer (see Provision at Flourish page) and believe that the EPU could enhance the education of a pupil.
Enquiries for further information are welcomed from schools by email - flourish@mitretrust.org.uk. We will get back to you and arrange a conversation. We would advise you to also have conversations with your family SENCO before making an application.
Schools are asked to consider the following points before completing the application form.
- Pupils remain on roll at the home school.
- Places are considered for a place using all the information provided. Visits to the home school by the Flourish team may be a part of the process.
- Places are awarded by a panel that includes MITRE leaders, family SENCOs, the local school behaviour and attendance partnership and Lowe's Wong staff.
- Schools will need to demonstrate the range of support already in place for the pupil.
- Schools must fully engage with families to agree to a placement at Flourish in advance of requesting a place.
- Statutory responsibilities for safeguarding and attendance remain with the home school.
- Placements are finite and the number and times of sessions will be agreed depending on individual need and capacity at Flourish.
- Transport to Lowe's Wong Juniors is the responsibility of the home school.
- If a pupil is entitled to funded one-to-one support at the home school, it will likely be that the member of staff will be asked to attend Flourish@LWJ with the pupil for their sessions.
- Schools will need to share detailed information about the pupil in advance of any placement and engage with daily communications when pupils are on placement.
- A service agreement will be put in place for any placement. This must be agreed by the home school, Flourish @LWJ and parents/carers.
- There is no charge to schools for accessing sessions at Flourish, although other costs (such as transport) may need to be met.

Application Form for Flourish EPU
This form should be completed by the SENCO or a senior member of staff at the registered school for the pupil. Submissions are not accepted directly from parent/carers. All the information submitted is treated confidentially and is only shared with members of the placement panel for Flourish EPU @LWJ.
The electronic form cannot be saved and therefore to support ease of completion a full set of the questions is available as download at the bottom of this page to aid preparation of responses in advance. Please note, word document should not be used instead of an electronic submission.
