Applications for Flourish @LWJ are welcomed from schools who have carefully considered the nature of the provision on offer and believe that the EPU could enhance the education of a pupil. They should understand the following points before completing the application form.

This form should be completed by the SENCO or a senior member of staff at the registered school for the pupil. Submissions are not accepted directly from parent/carers. All the information submitted is treated confidentially and is only shared with members of the placement panel for Flourish EPU @LWJ.
The electronic form cannot be saved and therefore to support ease of completion a full set of the questions is available as download at the bottom of this page to aid preparation of responses in advance. Please note, word document should not be used instead of an electronic submission.
